Fold out card for the Hospital Service Association of Roanoke. The card certifies that Joe. M. Francis and the following family members are entitled to hospital services when an in-bed patient in Blue Cross Hospitals regulations are met. Family members include: Florence C., Joe M. Jr., Harry L., James S., Davis B., and Robert W. Dates have been stamped beside Florence and Robert's names in a column at the right hand side - for Florence, it has been stamped 11-30-45, and for Robert 7-20-48. On the reverse of the card is the list of approved hospitals that are covered under the Blue Cross plan. Hospitals include the following: New Altamont Hospital in Christiansburg; Radford Community Hospital in Radford; St. Mary's Hospital in Martinsville; Stuart Hospital in Stuart; Jefferson Hospital, Leis-Gale Hospital, Roanole Hospital Assn., Shenandoah Hospital, Gill Memorial Eye, Ear, and Throat Hospital, and Burrell Memoral Hospital in Roanoke; Pulaski Hospital in Pulaski, St. Elizabeth's General Hospital in Pearisburg; Alleghany Memorial Hospital in Covington; C.& O. Ry. Employees Hospital in Clifton Forge; Jackson Memorial Hospital in Lexington; John Russell Hospital in Bedford; and Community Hospital in Danville. |
